Why Do I Need a Retainer?

Congratulations! You’ve done the hard work necessary to create your beautiful smile! You’ve carefully completed all the steps needed to reach the end of your orthodontic journey. Well, nearly all the steps. We can’t forget that last step which will ensure that all your hard work is rewarded.

When you first began orthodontic treatment, Drs. Lawrence Hier and Michelle Palmer decided on the best plan for straightening your teeth and perfecting your bite, whether you wore traditional braces, lingual braces, aligners, or other orthodontic appliances. And now that you’re finishing treatment, there’s one more option to consider—your retainer.

Why do I need a retainer?

While you’ve spent time in treatment, more has changed than just the position of your teeth. The periodontal ligament, the connective tissue that connects the teeth to the jawbone, is stretched as the teeth shift. The bone in your jaw changes, too, reforming and rebuilding around the roots of your teeth as they move to their ideal locations.

These changes happen because your braces or aligners apply gentle, constant pressure to move your teeth. When you’ve finished wearing these appliances, the pressure stops. Ligaments will try to return to their original shape, which can shift teeth back toward their old positions. And the rebuilding bone isn’t dense enough yet to stop teeth from shifting due to the normal, everyday pressures of eating, chewing, and smiling.

A retainer prevents your teeth from moving back, or “relapsing,” by giving your bones and ligaments time to stabilize and rebuild. The process takes months, so keeping your teeth in place as bones rebuild and grow denser is crucial. This is especially important for patients with more serious misalignments. Drs. Are there different kinds of retainers?

There are! Retainers can be removable or fixed, visible or nearly invisible, metal, plastic, or metal and plastic. Three of the most popular retainer options include:

  • Hawley Retainers—the traditional removable retainer, which uses a molded acrylic plate with wires attached to keep your teeth properly aligned and to hold your retainer in place.
  • Clear Plastic Retainers—a removable retainer made of custom vacuum-formed plastic, which fits over the teeth like a clear aligner.
  • Fixed Retainers—a small single wire bonded to the back of specific teeth, which holds them in place and prevents any movement.

How long do I need to wear a retainer?

There’s no standard answer to this question. Just like your retainer is custom-built to fit your individual teeth, the amount of time you’ll spend in that retainer depends on your individual needs. Retainers might be worn fulltime for months or years, be worn only at night after several months of daily wear, or be worn long-term to make sure your orthodontic work lasts.

Bracketology

Analyzing strong points, looking for potential problems, making comparisons—it’s bracketology time! Nope, not basketball (although we hear they have something similar), but a brief analysis of your orthodontic options when it comes to choosing a winning bracket.

If you’re getting braces, you’re probably already familiar with how they work—brackets are bonded to the teeth to hold an archwire, which provides gentle, controlled pressure to move the teeth into alignment. But within that basic bracket-and-wire system, there are several different bracket designs available to you at our Boynton Beach, FL orthodontic office. Let’s see what the scouting report has turned up on our final four, pointing out their distinct advantages as well as some potential mismatches.

Traditional metal brackets

Advantages:

  • Traditional braces with metal brackets are effective for more than just straightening teeth. They can be used to correct rotated teeth, differences in tooth height, and bite problems. For severe bite and alignment problems, traditional braces are most often the right choice.
  • Metal construction makes these brackets able to handle the controlled pressure needed to treat serious malocclusions.
  • Cost-effective. These are usually the least expensive option.

Potential Disadvantage:

Clear/Ceramic Brackets

Advantages:

  • Lack of visibility! Whether you go for clear brackets or brackets tinted to match your enamel, you’ll be keeping a low-profile with this choice.
  • Stronger and more stain-resistant than ever before, using the latest in ceramic, porcelain, or plastic materials.

Potential Disadvantages:

  • Not as durable. Unlike metal, these clear brackets can crack or break. If you play a contact sport, these might not be for you.
  • Some ceramic brackets are larger than other choices, so might be recommended only for the top teeth.
  • Clear or tinted brackets can be more expensive.

Self-Ligating Brackets

Advantages:

  • These brackets use a clip or trapdoor mechanism to hold your archwire without the need of bands. Ceramic options are available if you want an even more discreet appearance.
  • Can be more comfortable with less friction between wire and bracket.

Potential Disadvantages:

  • Self-ligating braces are generally more expensive.

Lingual Braces

Advantages:

  • Lingual braces use metal brackets, but they attach to the back of each tooth for almost invisible bite correction.
  • Custom-made. Lingual brackets can be designed and fabricated to fit your individual teeth perfectly.

Potential Disadvantages:

  • Trickier to clean because of their placement behind teeth.
  • Might not be suitable for a deep bite if there’s not enough clearance between top and bottom teeth.
  • Initial discomfort caused by the tongue’s contact with the braces when you speak and eat.
  • Custom-made brackets are more expensive.

The Best Brush of the Day

Imagine that you’re only going to brush your teeth once tomorrow. Don’t worry, we know you would never skimp on your dental hygiene like that, but let’s just pretend for a moment. When would be the best time to brush? When you wake up? During the day? Or perhaps before you go to bed?

Actually, whenever you choose to brush, you’ll receive important overall dental benefits as well as specific benefits tied to the time of day. Let’s explore your daily schedule to see why.

Brushing in the Morning

Brushing when you first jump out of bed produces several positive results.

  • Cleaning plaque from your teeth

Plaque is a sticky film made up of oral bacteria, food particles, and saliva. As you sleep, these oral bacteria multiply and produce acids which attack the minerals in your enamel, leaving weak spots which, over time, can become cavities. Brushing removes these bacteria and acids from your enamel before they cause serious harm.

Moreover, plaque hardens if it’s left undisturbed, turning into tartar in a relatively short time. And once plaque becomes tartar, it must be removed by a dental professional. Brushing first thing in the morning removes this plaque buildup and helps prevent tartar from forming.

  • Fresh breath

That bacterial growth we mentioned? It’s also responsible for morning breath. If nothing else, brushing when you wake up means greeting a fresh day with fresh breath, and that’s reason enough to pick up your brush in the morning.

Brushing During the Day

Brushing after meals and snacks also has a lot to recommend it.

  • “Leftovers” lead to cavities

Foods, especially foods rich in sugar and carbohydrates, are converted by oral bacteria into acids which weaken enamel and lead to cavities. When food particles remain in the mouth after a meal, bacteria have more time and more fuel to manufacture these acids.

  • Acidic foods also affect your teeth

If you have eaten something acidic, such as citrus fruits, sodas, or pickled anything, the acids from these foods can temporarily weaken the mineral strength of your enamel. But brushing immediately after eating or drinking acidic foods can damage weakened enamel. Better to rinse well with water and brush after half an hour or so.

  • When you wear braces

One of the first things you discover when you get your braces is that you might need to brush more often. In fact, it’s best to brush after every meal and even every snack while you’re in braces.

Why? First, because no one wants to smile with food particles sticking to brackets and wires. Even more important, though, the filmy plaque which sticks to your enamel can be harder to remove with those brackets and wires in the way. Since plaque causes weakened enamel and cavities, brushing thoroughly is more important than ever when you wear braces.

  • When you wear aligners

Wearing clear aligners means you don’t need to worry about food trapped in brackets or cleaning around wires. After all, you take them out when you eat. But this doesn’t mean you are home free. Brushing after every meal is also a good idea when you wear aligners.

Our teeth have an organic way to help wash away food particles, acids, and bacteria between brushings—saliva! Your aligners, while covering your teeth, decrease their exposure to saliva. It’s really important, then, to make sure you brush after eating. Otherwise, food particles and acids which remain on your teeth after eating are trapped in your aligners, increasing the risk of enamel erosion and decay.

Brushing at Night

Growing up, you probably received regular reminders to brush before bedtime—for several really good reasons:

  • Saliva production slows while you sleep

During the day, saliva helps to wash away food particles and neutralize acidity in our mouths. It also contains proteins and minerals which help keep tooth enamel strong. But as we sleep, saliva production slows dramatically, and our bodies can’t remove bacteria and acids as effectively.

  • Food particles fuel bacterial growth

If you haven’t brushed since morning, you’ve accumulated a whole day’s worth of food particles from meals and snacks. Remember, oral bacteria use the sugars and carbs we eat as fuel to produce the acids which attack our tooth enamel throughout the night.

  • Brushing helps prevent both of these problems

Brushing your teeth before bed not only cleans away the accumulated food particles of the day, but also eliminates the plaque and bacteria which would have a much easier time sticking to your teeth without that daytime saliva flow to wash them away.

So, When’s the Best Time to Brush?

In the morning, during the day, at night—there are solid advantages to brushing any time of day. The question isn’t so much when to brush as how often you should brush.

While many dental professionals consider brushing before bedtime as the most important brush of the day, brushing at least two full minutes, at least twice during a 24 hour period, is a necessity for basic dental hygiene, along with flossing at least once a day.

When you’ve been eating sugary snacks, when you’re showing signs of gingivitis or getting more than your share of cavities, when you want to reduce the chance of plaque and tartar buildup, or when you simply want to make sure you’re doing everything you can to maintain your overall dental health, brushing after meals is also highly recommended.

And when you wear braces or aligners, frequent brushing (and flossing) is the very best way to make sure your teeth stay clean and cavity-free.

Straight Talk about Braces and Oral Health

We’ll give it to you straight: it can be harder to keep your teeth their cleanest while you’re wearing braces. Food particles play hide-and-seek, plaque builds up around brackets, flossing is harder when you need to maneuver around wires. But keeping your teeth and gums healthy is even more important now that you’re wearing braces.

Why? Because when your braces come off, you want to enjoy the beautiful smile you’ve worked so hard for without worrying about discolored enamel, cavities, or swollen gums. Let’s look at some of the possible consequences when brushing and flossing are more challenging.

Decalcification

If you’ve noticed white spots around your brackets, you’re seeing the signs of decalcification, a common problem for those who wear braces.

Decalcification begins when plaque collects on the enamel around your brackets. The bacteria in plaque produce acids. These acids eat away at the minerals which keep your enamel strong, minerals like calcium and phosphorous. Places on the enamel where erosion takes place are left weakened and discolored.  Eventually, these weak spots can lead to . . .

Cavities

When plaque sticks around, whether near your brackets or anywhere on your teeth, it provides the perfect conditions for decay to develop. Left untreated, bacterial acids continue working away at decalcified spots in your enamel. This continuing erosion causes these surface spots to expand, grow deeper, and become cavities.

If you’re having trouble with built up plaque, and brushing isn’t doing the job for you, your dentist can remove it with a professional cleaning. Getting rid of plaque is healthy not only for your enamel, but your gums as well.

Gum disease

When plaque and tartar collect around the gumline, they irritate delicate gum tissue. This irritation causes gingivitis, or early gum disease. And, while young people rarely suffer from serious gum disease, the pain, redness, bleeding, swelling, and bad breath caused by gingivitis are not anyone’s life goals!

Brushing and flossing are essential to keeping your enamel and gums plaque-free. But even if you brush more often, it’s not as easy as it once was now that you have to work around and between brackets and wires. Luckily, there are lots of tools out there to help you get your teeth, gums, and braces their cleanest.

  • Orthodontic toothbrushes

Special brushes designed just for braces can help you work around brackets and wires. V- or U-shaped bristle formations let you brush around and over your braces. Curved bristles can fit under wires. Smaller brush heads let you reach those hard-to-reach places.

  • Electric toothbrushes

Many people find these brushes can clean more easily and effectively, especially when wearing braces. Tapered orthodontic brush heads are available, and, if you’re a heavy-handed brusher, there are models which alert you if you’re brushing too hard—protecting your braces and your enamel.

  • Orthodontic floss

Special orthodontic flosses can help you do the tricky job of fitting floss behind your wires and between your teeth, or use a floss threader, which helps guide uncooperative floss into tight spaces.

  • Water flossers

With their pulsing streams of water, water flossers can reach spots where regular brushes and flosses just can’t comfortably fit. There are even flossers available with special orthodontic tips.
